This information may include, for example, your age, date of birth, place of birth, occupation, presenting symptoms, medical history and family history. SWHN may also collect and use personal information about you including your telephone number and address for the purpose of contacting you in relation to an appointment or to send you any materials either prior to or following an appointment.


Furthermore, SWHN may collect and use personal information about you, for the purpose of distributing information to you, with your consent, including:


  1. your name via the SWHN Facebook page, and/or
  2. your email address(es) via the SWHN Website for the SWHN e-newsletter.

SWHN does not collect click stream data or cookies about your web activities.
